​
Our e-Credentialing process requires these documents to be uploaded to your profile:

• Copy of current medical registration, including Wyvern Private Hospital provider number if already applied via PRODA
• Cover letter noting scope of practice applying for
• Copy of current medical indemnity insurance showing relevant specialty covered and expiry date
• Copy of CV
• Copy of Qualifications
• Contact details of 3 professional referees
• Copy of vaccination record
• Copy of Working with Children’s Check
• Copy of Police Check
• Copy of 100 points of identification, such as passport, drivers license, Medicare card

​Next steps

• After receiving your completed application, the application will be reviewed by the CEO of Wyvern Private Hospital
• Applications will be referred to the Medical Advisory Committee for consideration
• If endorsed by the Medical Advisory Committee, your application will be referred to the Wyvern Board of Directors for ratification
• We will then notify you of the outcome of your application
